To claim, click HERE for the Claim Form and submit it together with copies of

1. BI 1663 Notification of Death from the hospital or mortuary – be sure to make copies before going to Home Affairs for the Death Certificate, copies do not need to be certified
2. Death Certificate from Home Affairs
3. ID/Passport of the Deceased, if it is the new ID card we only need a copy of the front
4. ID/Passport of the Claimant – Main Member or Beneficiary as listed on application/latest amendment.
